If you are one of those people that can hardly wait to receive all your documents so you can file your tax return - have I got a deal for you!
One of those documents is the 1095-C. It is a requirement of the Affordable Care Act (ACA.) It is produced by the City and tells you, and the IRS, whether or not you were eligible for and enrolled in the City's medical insurance in 2018.
The forms are compiled, printed and sent to employee mailing addresses by the end of January. However, you have the opportunity to elect to receive it sooner by consenting to receive an electronic copy (PDF) that you can print out yourself.
Attached is a job aide to give your consent through Employee Self Service (ESS) in PeopleSoft.
1. Once you give consent it remains in affect until you withdraw it.
2. Giving consent to receive an electronic version of the form means one WILL NOT be mailed to you.
3. An email will be sent to your City email address notifying you when your form is available.
4. Call the Benefits Office at 768-3758 if you have questions.
